Petition for Certificate Program 2011

A Petition for Certificate must be submitted to Admissions and Records by March 16, 2011. This deadline has been extended to March 16 and this is a FINAL deadline. Select your Certificate Program from the list below and print out and complete the petition. (For your convenience the petitions are made available in both Microsoft Word format and pdf format. Choose one or the other.) Students MUST meet with a counselor prior to submitting their petition.
