The Student Learning Outcomes and Assessment Cycle: Reporting Forms

SLO REPORTING FORM for COURSES (INSTRUCTIONAL)  NEW and IMPROVED!!

There's now only one form for all portions of the SLO Assessment Cycle!

Directions to fill out the form:

• Download and complete the SLO Reporting Form to report Student Learning Outcomes, assessment data, and reflections on the data.
• Use one form per course. Complete the form for any number of SLOs and add additional data and reflections as they occur throughout the term.

• After completion, change the file name to DEPT_Course number_term and submit the form as an e-mail attachment to canslo@smccd.edu and your Division Dean.
