depressed, inability to concentrate, spaced-out, expressing suicidal thoughts) where
you are concerned for a student

This report will go to the Vice President of Student Services Office (VPSS) and will
be reviewed by the CARES Team (The Vice President, a member of Public Safety, a Personal
Counselor, and an Academic Counselor)
You, the reporter, will not be revealed to the student of concern
This form is to report harassment or discrimination on the basis of sex (sexual misconduct,
sexual harassment, sexual violence)
If there is an emergency of sexual violence, contact Public Safety at 650-738-7000
or 911 (depending on the situation)
This report will go to the Title IX Coordinator and will be reviewed by the Title
IX Team (The Dean of Counseling, Student Life and Leadership Member, Athletics Member,
and Student Services Manager)
This form is to report harassment or discrimination NOT based on the basis of sex
(sexual misconduct, sexual harassment, sexual violence) or a mental health concern
