Faculty and Staff Student Referral to the PCC:
- If you have a concern about a student you can complete a referral to the Personal Counseling Center. If a student lets you know that they are experiencing mental health symtopms or personal issues please complete this form so we can support the student and provide linkage to services. CARES ( Cañada Assesment, Response, and Evaluation)
- In your role as a staff or faculty member, you may come in contact with students who are distressed, disruptive or dangerous. A student might directly confide his or her concerns to you, or you might infer that he or she is in distress by observing the student's behavior. CARES Information
- This form is to be used to report behaviors of concern, for example, distressed behavior where you are concerned for a student. If there is an emergency or the behavior is very disruptive or dangerous, contact Public Safety or 911 (depending on the situation). CARES Report
